- Offer new places for children with additional needs.
- Strengthen inclusive practice across local settings.
How We Work
- Training and professional development for staff.
- Development of future Hubs in other areas.
- Shared locality responsibility and collaboration.
- Meeting the increasingly complex needs of pupils accessing mainstream education.
- Ensuring every child thrives within their local school community.
- Small, specialist Hubs embedded within mainstream Trusts.
- Fully supported by the Special Partnership Trust.
DCC/SPT Year 6-7 Transition Project

We are dedicated to ensuring that every student receives the support they need as they move from primary to secondary education. This project provides specialist advice, guidance, and practical support for children, families, and school staff.
How We Work
- Shared learning walks to observe and reflect on inclusive practice.
- School tours and transition visits tailored for pupils with SEND.
- Open events to share good practice and strengthen professional networks.
- Reviewing and refining transition processes for greater consistency.
- Targeted transition activities and in-school support for pupils.
- Staff CPD sessions to enhance SEND provision and confidence.
- Strategic deployment of staff and planning for effective transition.
- Parent workshops to ensure families feel informed and supported.
- Curriculum planning and bridging work for continuity of learning.
- Collaboration with feeder primary schools for strong information sharing.
- Creating sensory-friendly environments to reduce anxiety and promote inclusion.
One-to-One Outreach
Our one-to-one outreach offer is commissioned by Devon County Council (DCC) for children with EHCPs who experience significant barriers to attendance, engagement, or inclusion.
This is not a request-based offer for settings. The need for support is identified through locality teams, and all current cases are funded by Devon County Council.
Please do not contact SPT directly to request 1:1 outreach. If you have concerns about meeting the needs of a child or young person in your setting, please book a SEND Support Consultation or attend a North Devon SEND & Inclusion Drop-in.
